What you need to know about this position:

  • Under the supervision of the Manager, Population Health and Value Based Programs, the Ambulatory Program Coordinator provides oversight of high risk pediatric population at the physician practice level. The Ambulatory Program Coordinator will be responsible for assisting the Manager, Population Health at the practice level to close gaps in care utilizing EPIC.
  • The Ambulatory Program Coordinator will be responsible for communicating and working with the physicians, families and office to reduce emergency room visits and unnecessary hospitalizations and performing Transitional Care Management of the pediatric population.
  • The Ambulatory Program Coordinator position has an hourly base rate of $29.66 that is commensurate with experience.
  • Our Ambulatory Program Coordinator has an 8:00am-4:30pm schedule, Monday-Friday.

What is required for this position:

  • Associates of Science in Nursing (ASN) required.
    • Bachelors of Science in Nursing (BSN) preferred.
    • 5 Years of clinical experience preferred.
    • Pediatric Experience preferred
  • Current Indiana License Registered Nurse (LRRN) required.
  • Basic Life Support (BLS) certification required.
