What you need to know about this position:
- The Community Health Worker will work as part of a perinatal support team to support families during pregnancy and during baby’s first year and a half. This support will include education on the health and development of the infant, referral to providers and connection to community resources as appropriate.
- The Community Health Worker will engage in assessment of assets and gaps in the community of focus, and provide outreach and support within this community to ensure equitable access to perinatal care for families.
- This position will identify and build relationships with community resources, attend community events and activities and increase the community’s knowledge regarding women’s health, pregnancy, breastfeeding, infancy, parenting and early childhood.
- The base hourly rate for this position is $21.13. Individual compensation is determined for this position through years of directly relevant experience. The hourly compensation is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
- This part-time position would require you to work 20 hours a week, variable days and shifts between 8:00AM-5:00PM, Monday through Friday.
What is required for this position:
Education and/or Experience
- High School Diploma or GED required; Associate’s or Bachelor’s degree preferred
- Prior work experience in administration, community services, or health preferred
Certifications, Licenses, Registrations
- Community Health Worker certification required within 12 months of hire
